Step 1: Choose a plan type
Our most popular plans feature a $25 or $35 copay for Primary Care Physican (PCP) office visits, along with a deductible and coinsurance for most other types of services. Plus, you have a choice between two different prescription drug benefit plans and optional maternity and/or dental coverage.

Step 2: Choose your calendar year deductible
The deductible is the amount you pay out of your pocket before the plan begins to pay benefits. We offer five deductible amounts, ranging from $500 - $5,000.

Step 3: Choose your coinsurance percentage
The coinsurance percentage is the percentage of that the plan pays after you have met your calendar year deductible. We offer a standard 80 percent coinsurance on most plans.
